The doctors and staff at Hanwell Health Centre in Hanwell are proud to offer the highest standard of patient-centred healthcare.
We run clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, childhood vaccinations and well-person check-ups.

NHS Health Checks (aged 40 - 74)
The NHS Health Check programme is an initiative which offers preventative checks to all those aged 40-74 to assess their risk of vascular disease (heart disease, stroke, diabetes and kidney disease) followed by appropriate management and interventions.
Please complete a questionaire which you can get from reception, check your blood pressure in the corridor and hand back the completed form.

NB. This does not apply if you have already been diagnosed with any of the above conditions.
